An opportunity currently exists for an experienced person to join our team as Service Coordination Officer. This short term contract (Oct 2016 - April 2017) role will be responsible for developing and implementing strategies in relation to service coordination and chronic disease management. This will be achieved by building the capacity for consumers to experience safe, effective and client centred healthcare through integrated and coordinated systems.

Additionally the Service Coordination Officer is responsible for the development of stakeholder collaboration to ensure enhanced engagement and consistent and effective service coordination across five local government areas (Bayside, Glen Eira, Kingston, Port Phillip and Stonnington).
