Our partnership of community and primary care agencies work collaboratively to:
- Improve coordination of services through use of common tools, principles and practices
- Improve pathways of care and access to services
- Support integrated chronic disease management
The SMPCP Service Coordination and Integrated Care (SCIC) Steering Group leads the Service Coordination and Integrated Client strategy and provides direction for the implementation of the SMPCP SCIC Operational plan.
