Since their inception in 2000, both Inner South East Partnership in Community Health (ISEPiCH) and Kingston Bayside Primary Care Partnership (KBPCP) delivered a broad range of projects which enhanced the health and wellbeing outcomes across the service sector within their respective catchments. The two PCPs merged in July 2014 to form Southern Melbourne Primary Care Partnership.
