Primary Care Partnerships transitioned to Local Public health Units

From 1 July 2022, all Victorian Primary Care Partnership program funding transition to the Local Public Health Units. Primary Care Partnership functions for the South East Melbourne region is now being delivered through the South East Public Health Unit (SEPHU).

All previous SMPCP functions have moved to SEPHU. Past project information is still available on this website but future enquiries should be addressed to SEPHU.

The Kingston SRS Network meets to share information, discuss issues and collaboratively problem solve to improve service provision to the residents of pension level SRSs

Who can attend?
Any staff member from agencies that provide services to pension level SRSs in the City of Kingston as well as interested proprietors and staff.

Why attend?

  • Network with other health professionals working in SRSs in the City of Kingston
  • Develop inter-agency and inter-worker relationships to support collaborative service delivery to clients

Meetings:
Meetings are held quarterly in February, May, August, and November
