Primary Care Partnerships transitioned to Local Public health Units

From 1 July 2022, all Victorian Primary Care Partnership program funding transition to the Local Public Health Units. Primary Care Partnership functions for the South East Melbourne region is now being delivered through the South East Public Health Unit (SEPHU).

All previous SMPCP functions have moved to SEPHU. Past project information is still available on this website but future enquiries should be addressed to SEPHU.

The Clinician's group is made up of clinicians from the four Community Health Centres located within the catchment of SMPCP.
Disciplines represented are:

  • Physiotherapy
  • Occupational Therapy
  • Counselling
  • Podiatry
  • Nursing
  • Allied Health Assistance
  • Speech Pathology
  • Social Work

The group works closely with Consumer and Community members and is supported by managers and coordinators of the four Community Health Services.

The group was established following an Assessment of Chronic Care Illness survey in three Community Health Services within the catchment.

Purpose:
The purpose of the group is further build in and sustain practice change as part of the DHHS Chronic Illness Care strategy. This is achieved through a collaborative method that uses the collective insight, expertise and experience of members to problem solve and make improvements.
