Primary Care Partnerships transitioned to Local Public health Units
From 1 July 2022, all Victorian Primary Care Partnership program funding transition to the Local Public Health Units. Primary Care Partnership functions for the South East Melbourne region is now being delivered through the South East Public Health Unit (SEPHU). All previous SMPCP functions have moved to SEPHU. Past project information is still available on this website but future enquiries should be addressed to SEPHU. Enquiries.....
