Primary Care Partnerships transitioned to Local Public health Units

From 1 July 2022, all Victorian Primary Care Partnership program funding transition to the Local Public Health Units. Primary Care Partnership functions for the South East Melbourne region is now being delivered through the South East Public Health Unit (SEPHU).

All previous SMPCP functions have moved to SEPHU. Past project information is still available on this website but future enquiries should be addressed to SEPHU.

Executive Officer

With a passion for prevention, Michelle has over 30 years experience working in various roles within the health sector including more than 12 years in the Primary Care Partnership environment. Michelle has a background as a dietitian in addition to qualifications in information management and communications. 

Michelle is now focused on stakeholder engagement and management as well as executing financial and leadership responsibilities for Southern Melbourne Primary Care Partnership. She also leads the SMPCP Elder Abuse Prevention Network project.

Tracey Collins

Partnership Lead - Prevention
Tracey joined SMPCP in March 2019, bringing to the role 20 years of experience in community health across the southern metropolitan region. Her work in the area of gambling harm prevention has developed strong skills in project coordination and community engagement, featuring collaboration across diverse groups and sectors. Tracey has a secondary school teaching background in Health & Physical Education.

Ailsa Gregory

Partnership Lead - Service Integration & Access

Ailsa has vast experience in Chronic Disease as both a clinician and program manager. While she has worked across multiple projects she has a strong passion for diabetes, GP engagement, training and advance care planning. Her key focus is to improve health service outcomes that will enhance all aspects of the patient’s life experience across the health system. 

Her passion for good health has led her to embrace cycling and stand up paddle boarding which she finds extremely exhilarating and strenuous while providing her time to relax and contemplate life.

Partnership Lead - Consumer Engagement
Ken has a background in teaching as well as extensive experience in community services delivery from youth homelessness, out of home care/leaving-care/fostercare, alcohol and other drug treatment services, juvenile justice and disability services through to creative areas of establishing digital arts and media opportunities for young people. He joined the SMPCP team in early 2015 and believes that building better health and wellbeing comes through collaborative coordination and planning for an effective service system.

Chris Edmonds

   

Chris Edmonds

Partnership Lead
Chris is extremely passionate about Aboriginal and Torres Strait Islander health having worked in Close the Gap funded programs with both Medicare Local and Primary Health Network. This experience motivated her join SMPCP as the Aboriginal and Torres Strait Islander Access Project Officer role in the hope of continuing to work toward closing the health gap between Aboriginal and non-Aboriginal Australians. Chris's role has now expanded to support PCP initiatives in the areas of health literacy and elder abuse. Chris worked for more than 12 years with Divisions of General Practice coordinating programs such as mental health and chronic disease. Chris holds qualifications in social work and education.

Business Support
Tracey holds a Bachelor of Business and a Masters in International Management. She has spent her professional career working in various managerial sales roles within the corporate arena. After taking a career break to raise her family, Tracey wanted to take a different career path. She is passionate about community health and giving back to the community and this interest led her to join the SMPCP in early 2018. Tracey brings strong administrative, customer service and communication skills to the team.
