Staff acknowledge National Close the Gap Day

Close the Gap 2017

National Close the Gap day was on Thursday 16 March and to mark the occasion SMPCP held an event at the new Aboriginal and Torres Strait Islander Gathering Place (housed within the Mordialloc Life Saving Club building).

Supported by Central Bayside Community Health Services, Connect Health & Community, Inner South Community Health and the Salvation Army Crisis Services, the event was very successful. Speakers Mark Dreyfus MP and Mark Williams struck a chord with those attending and Caroline Martin was inspirational in delivering the welcome to country as well as emceeing the event.

Having identified that our local Aboriginal and Torres Strait Islander community is a young and growing one, we have started up a Koorie Mums and Bubs group to enhance community engagement.

SMPCP receives ADMA recognition for Model of Care Project

The SMPCP Community Health Integrated Chronic Disease Collaborative recently presented their Community Health Model of Care at the Australian Disease Management Conference.

In keeping with the conference theme Person Centred Healthcare: Achievements & Challenges and co-design principles, the Model of Care was developed with consumers, clinicians and managers from all four Community Health Services in the SMPCP catchment. The Model provides care options for clients when they are self-managing and when they need comprehensive assistance and care coordination. It has ‘built in’ fluidity enabling the client to move through care options dependent on their changing health status.

Service Coordination Officer (0.6 EFT)

An opportunity currently exists for an experienced person to join our team as Service Coordination Officer. This short term contract (Oct 2016 - April 2017) role will be responsible for developing and implementing strategies in relation to service coordination and chronic disease management. This will be achieved by building the capacity for consumers to experience safe, effective and client centred healthcare through integrated and coordinated systems.

We are excited to announce that, after the success of our inaugural Fluro Friday, we will be running the event again in 2016 to celebrate Mental Health Week. So, find your fluro socks, don a Hawaiian lei, swipe on some coloured zinc or borrow a high-vis vest to come along and enjoy a fun-filled day.  

Did you know that each year, approximately one in every five Australians will experience a mental illness? Our fun Fluro Friday event helps raise awareness of this important health issue in a positive way and helps make the invisible - visible.
